What This Actually Is

Knee osteoarthritis is degeneration of the joint surface, along with changes in the bone underneath, the lining and the surrounding soft tissue. It is not simply “wear and tear,” and it is not a straight line from diagnosis to replacement.

The important and under-appreciated fact: how bad your X-ray looks correlates poorly with how much pain you’re in. Plenty of people have severe imaging findings and little pain, and the reverse. Which means pain and function can improve without the X-ray changing at all.

Much of what hurts around an arthritic knee isn’t cartilage — it’s the surrounding tendons, the fat pad, the bone just under the surface, and a quadriceps that has quietly wasted.

The Real Problem

Why It Hasn’t Gone Away On Its Own

The knee is loaded every time you stand up, take a stair or walk to the car. There’s no realistic way to unload it enough for tissue to settle on its own.

Pain makes you use the leg less, the quadriceps weakens, the joint loses its shock absorber, and load distribution gets worse. This is the loop that turns a manageable knee into a bad one over a few years.

The standard path — anti-inflammatories, a cortisone shot, wait, repeat — manages pain without addressing the muscle loss or the load problem underneath it.

Nobody promises you a new joint surface. What we’re aiming for is less pain, more capacity, and a knee that lets you do the strengthening that changes the long-term trajectory.

If you’re heading for a replacement anyway, getting stronger first measurably improves how that surgery goes. That work isn’t wasted either way.

Are You A Candidate?

Shockwave is genuinely good at a narrow set of problems. It is not good at everything, and candidacy is determined by examination, not by a website.

 Diagnosed knee OA with pain limiting stairs, hills or walking distance

 You’ve been told you’re “too young” for surgery

When It Isn’t The Answer

 A locked knee, or true mechanical locking and catching — that needs an orthopaedic opinion

 A knee that’s hot, swollen and acutely inflamed — that needs a diagnosis first

 End-stage arthritis with bone-on-bone deformity and constant rest pain, where a replacement is genuinely the right answer — we’ll say so

 Suspected inflammatory arthritis or gout, which are medical problems

 Active infection, open wound, tumour in the area, or pregnancy
