What This Actually Is
The rotator cuff is four muscles and their tendons wrapping the ball of your shoulder. In calcific tendonitis, calcium hydroxyapatite crystals deposit inside one of those tendons — most often the supraspinatus.
Nobody is certain why it starts. What’s clear is the pattern: a long quiet phase, then a formative and resorptive phase that can be genuinely severe — the kind of shoulder pain that has people in urgent care at 2am.
Chronic non-calcific cuff pain (tendinopathy, partial thickness wear, subacromial irritation) is the other large group we treat, and it behaves like the tendon problems elsewhere on this site: degenerative, poorly supplied, stalled.
The Real Problem
Why It Hasn’t Gone Away On Its Own
A calcium deposit is a physical thing inside the tendon. No amount of exercise, stretching or anti-inflammatory will dissolve it, which is why so many people cycle through months of treatment with an unchanged X-ray.
The shoulder is hard to rest and even harder to sleep on. Night pain disturbs sleep, poor sleep raises pain sensitivity, and the cycle feeds itself.
Pain inhibits the cuff, the cuff stops controlling the joint, the mechanics get worse and the tissue gets more irritated. By the time most people come in, there are three problems stacked on each other.
The Treatment
How Shockwave And EMTT Address It
Shockwave delivers high-energy acoustic pressure waves into the tissue. The body reads that controlled stimulus as a fresh injury and reopens the healing process that stalled out. Here’s what that means for this specific problem.

Direct action on the calcium deposit
Focused shockwave is one of the very few non-surgical options with a plausible, documented mechanism against calcific deposits — the energy helps fragment and mobilise the calcification so the body can resorb it. This is the strongest single case for shockwave anywhere in the body.
Depth that matters here
The supraspinatus tendon sits under the acromion, several centimetres down. Focused shockwave is designed to put its energy at that depth rather than dumping it in the surface tissue.
Restarts repair in the tendon itself
For non-calcific cuff tendinopathy, the same mechanism as elsewhere applies: new vessel growth, collagen turnover, reduced pain signalling.
EMTT across the whole joint
Shoulders are one of the clearest cases for adding magnetic transduction. It treats a large volume — cuff, capsule, surrounding tissue — and often takes the edge off the night pain first.
What To Expect
What A Course Of Treatment Looks Like
Calcific shoulders often need three to five focused sessions a week apart, and we usually recommend a follow-up X-ray to see what’s actually happened to the deposit.
Expect the treatment itself to be uncomfortable on a calcific deposit. We control the energy and build up, and it’s twenty minutes, not an hour — but we won’t pretend it tickles.
Cuff strengthening runs alongside from early on. A deposit that resorbs into a shoulder with a weak, uncontrolled cuff is a shoulder that will hurt again.

Are You A Candidate?
Shockwave is genuinely good at a narrow set of problems. It is not good at everything, and candidacy is determined by examination, not by a website.
Shockwave Is Usually A Good Fit If
✓ An X-ray or ultrasound showing calcific deposit in the cuff
✓ Shoulder pain lasting more than three months
✓ Night pain and difficulty sleeping on that side
✓ You’ve had physical therapy or a cortisone shot without lasting change
✓ Surgery has been raised and you’d like to try something first
When It Isn’t The Answer
✗ A full-thickness cuff tear with real loss of function — you can’t lift the arm at all
✗ A suspected frozen shoulder in the inflammatory phase — that’s a different problem and shockwave isn’t the first answer
✗ Instability or repeated dislocation
✗ Pain that’s referring from the neck rather than arising in the shoulder
✗ Active infection, open wound, tumour in the area, or pregnancy
